Dear Parents,

It is already time to register your child(ren) for Christian Formation Classes at St. Ann’s for the 2007-2008 school year. Please fill out the following form for your family and return to the parish office. All children in Kindergarten through 11th grade should be included. The fee for participating in Christian Formation classes will be $25.00 if Registration and payment is received before Friday, June 1, 2007 and $30.00 if received after Friday, June 1, 2007 (with a maximum of $100.00 per family).  If these fees present a financial hardship for your family, please talk to a member of the Christian Formation Staff or Father Chris.  We want to ensure that everyone is able to participate. Christian Formation Classes begin in September. Grades Kindergarten through 6th meet on Sunday mornings at 9:00am and Wednesday nights at 5:45pm. The Youth Session 7th - 11th graders will meet at 11:30 after mass on Sunday until 1 pm. All parents with children in the Christian Formation program are expected to volunteer, please indicate, on this form, where you will volunteer. Please use the below links to perform registration by downloading a form, to print, fill out and mail to St. Ann's Office or if you use the word document you may use it as an attachment in email, no later than September 1, 2006.
